Diane Schaaf, RN, Memorial Scholarship by Upland Hills Health: Upland Hills Health established this scholarship in memory of Diane Schaaf who was a nurse at UHH. Diane cherished her family and was dedicated to nursing. This scholarship will provide financial assistance to a student who has completed 34 credits (comparable to second year status) in the A.D.N. Program. To be eligible to apply, applicants must have graduated from one of the following high school districts: Dodgeville, Mineral Point, Highland, Iowa-Grant, Barneveld, Pecatonica, Mount Horeb, River Valley, Boscobel, or Darlington. The recipient must also be a full-time student (12 credits or more). Applicants may only receive the scholarship one time and if employed by Upland Hills Health, Inc., during the scholarship application process, applicants will not be eligible to apply if he/she is receiving tuition assistance directly from Upland Hills Health, Inc. Recipient must maintain a cumulative GPA of 2.5.
How To Apply
Who Can Apply For Scholarships?
Current Southwest Tech students who are enrolled in a minimum of at least 12 credits each semester. Exceptions to this rule are as follows:
Students who are transferring credits from another institution of higher education in order to complete a program are eligible to apply for Southwest Tech Foundation scholarships designated for full time students. In order to qualify, the following criteria must be met. Students must be enrolled in a minimum of 18 credits in the current academic year and must carry a minimum of 8 credits each semester. The student must have completed and documented with student services a minimum of 6 transferable credits into the program they have designated on the scholarship application.
Students with a documentable disability who are enrolled in less than 12 credits must have a viable plan on file with the Support Services Office.
Students of color who are enrolled in at least 3 credits each semester and are a member of the C.L.A.S.S. Club (applies only for the C.L.A.S.S. Club Scholarship).
Associate Degree – Nursing student carrying at least 6 credits each semester (applies only to the SWTC Student Nurses Assoc. Scholarship).
Supporting Materials Required With Application:
Two recommendation forms are required. Completed forms must be from someone not related to the applicant. Only two forms will be accepted. If more than two forms are submitted, only the first two received will be accepted. Recommendation forms must be returned to the Financial Aid Office or to the Foundation Office in a sealed envelope with the reference source’s signature across the sealed portion of the envelope. (Forms available at the Financial Aid Office, the Foundation office, or on the College website).
If applicable, Child Care Certification slip must be completed and returned to the Financial Aid Office or to the Foundation Office. (Forms available at the Financial Aid Office, the Foundation office, or on the College website).
Southwest Tech transcripts will be provided by the Foundation office.
Transcripts from other college(s), if applicable (applicant's responsibility).
Applicant Is Responsible For The Following:
